Patients who are on any contraceptive pill can request their repeat prescription using the Contraceptive Pill Review.
Alternatively, there are printed versions in the waiting area and also patients can request one to be printed for them from reception.
- Anyone under the age 16 or above 50 should have a face to face appointment to assess needs and eligibility/need for contraception
- Patients who hasn’t had the pill for 3 consecutive months up to the point of request
- If the form is incomplete eg. no Blood Pressure/height/weight entered, patients will be urged to complete the form which can be done over the phone or asked to fill a new form to send in.
The filled forms can be returned to the surgery via post, handing into the reception or by posting it via the prescription box in the entrance of the surgery.
Filled forms are scanned onto Docman and put into the daily post/task list via patient’s own GP.
